2012 Central South Regional Team Tournament Registration

Tournament will be held Saturday, February 25, 2012 at Broadmoor JHS, 501 Maywood, Pekin, IL

Team Managers or Coordinators: Please complete one entry form PER team.

This form is Part One of the Registration process. Each team is required to provide an Appraiser and a Volunteer:

Payment for Tournament is Part Four, the final phase, of the registration process. The Fee is $70.00 if sent by January 18, 2012. If received AFTER that date, the fee is $100.00. Sometimes a team chooses a special name to call themselves, this is the opportunity to let the Region know what that is

Each team must designate which Team challenge they will be solving for the Tournament. Please fill in the blank with one of the following:

Every Team requires at least one current email that is checked frequently in order to receive communications and information pertaining to participation in ILDI Tournaments

The Region needs a phone number where the TM can be reached, primarily for urgent contact.

Please be sure to note any special scheduling or other considerations that should be taken into account regarding this team or one of its members (e.g. team member with a disability, TM has more than one team, etc)
