2012 Far North Regional Team Tournament Registration

Tournament will be held Saturday, March 3rd, 2012 at Juarez Middle School in Waukegan IL, the same location as in 2011

Payment for Tournament is $70.00 if sent by January 18, 2012. If received AFTER that date, the fee is $100.00. Payment should be mailed to: Far North ILDI c/o Todd Underwood, Treasurer, 283 S. Collins St.,South Elgin, IL 60177. Problems or questions, please email fn-treasurer@illinoisdi.org

Team Managers or Coordinators: Please complete one entry form PER team.

This form is part one of the Registration process. Each team is required to provide an Appraiser and a Volunteer.

Either school or name team membership was purchased under

Number assigned by DI HQ when teampak was purchased

Sometimes a team chooses a special name to call themselves, this is the opportunity to let the Region know what that is

Each team must designate which Team challenge they will be solving for the Tournament. Please fill in the blank with one of the following:

This is the level the team will compete in at Tournament. Please fill in the box with one of the following:
Elementary
Middle
Secondary (High School)
University
Rising Stars

Every Team requires at least one current email that is checked frequently in order to receive communications and information pertaining to participation in ILDI Tournaments

The Region needs a phone number where the TM can be reached, primarily for urgent contact.

Please be sure to note any special scheduling or other considerations that should be taken into account regarding this team or one of its members (e.g. team member with a disability, TM has more than one team, etc)
