2012 North Central Regional Team Tournament Registration

Tournament will be held Saturday, March 3rd, 2012 at Hadley JHS, 240 Hawthorne Blvd, Glen Ellyn, IL 60137

This Team Registration Form is only Part One of the Registration process. Each team is also required to provide an Appraiser and a Volunteer:

Payment for Tournament is Part Four of the registration process. The fee is $70.00 if sent by January 18, 2012. If received AFTER that date, the fee is $100.00. Payment should be mailed to: North Central ILDI c/o Diana Dignan, Regional Director, 875 Smith St, Glen Ellyn 60137. Email if you have any problems or questions

This is the level the team will compete in at Tournament. Please fill in the box with one of the following:
Elementary
Middle
Secondary (High School)
University
Rising Stars

Every Team requires at least one current email that is checked frequently in order to receive communications and information pertaining to participation in ILDI Tournaments

The Region needs a phone number where the TM can be reached, primarily for urgent contact.

Please note any special scheduling or other considerations that should be taken into account regarding this team or one of its members (e.g. team member with a disability, TM has more than one team, etc)
